HOSPICE RN CASE MANAGER

Are you a Registered Nurse (RN) looking to get into Hospice or an experienced Hospice RN looking for a change?

We have partnered with a compassionate hospice care provider serving Philadelphia, Bucks, and Montgomery Counties. We're seeking an experienced Hospice RN or a motivated Home Care RN who is looking to gain knowledge and experience in Hospice.

JOB SUMMARY:

• Coordinate hospice care by working with other team members as well as with the patients/families to ensure needs are met.
• Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
• Perform evaluations for hospice care along with informational visits.
• Utilize the nursing process and best practices to provide professional care.
• Coordinate patient care services with other members of the care team as well as contracted services when needed. 
• Update and document care plans and changes in the plan of care. Consult with the attending physician and Hospice Medical Director when needed. 
• Actively participate in QAPI meetings and activities as well as staff development meetings.

REQUIREMENTS:

• Must be licensed RN in the state of Pennsylvania
• 1 year of hospice experience is preferred
• Must be a licensed driver with an insured, reliable vehicle
• Must have covid vaccinations
