Schedule: Monday - Friday, 8a-4:30p

Join a thriving Senior Healthcare program in Reading, PA, where they are dedicated to enhancing the quality of life for our elderly population. We are looking for a compassionate and skilled Occupational Therapist to support seniors in achieving their personal health goals.
Position Overview:
As a licensed Occupational Therapist, you will play a vital role in evaluating and reassessing participants, developing tailored treatment plans, and providing therapeutic interventions to help them maintain their independence at home. You will collaborate with a supportive team, including two Certified Occupational Therapy Assistants (COTAs), ensuring a comprehensive approach to participant care.
Key Responsibilities:
- Conduct evaluations and reassessments upon referrals from the Interdisciplinary team.
- Develop individualized treatment plans focused on enhancing participants' functional abilities.
- Create and implement therapeutic exercise programs and assess progress toward goals.
- Perform home safety assessments for new participants and update as needed.
- Complete care plans during assessments, including enrollments, semi-annual reviews, and as requested by families or caregivers.
- Evaluate participants' home environments for safety and equipment needs, providing in-home assessments when necessary.
Qualifications:
- Valid Occupational Therapy license in Pennsylvania.
- Minimum of 1 year of experience working with the frail or elderly population.
- Demonstrated clinical competency with geriatric clients.
- Management experience in rehabilitation or related fields is a plus.
- Valid driver’s license and reliable transportation.
