RN Case Manager

Responsibilities include but not limited to:

• Identifying medical needs for individual members, and setting care priorities accordingly

• Developing ongoing Plans of Care, in partnership with our client’s interdisciplinary teams

• Interpreting, recording, and acting on diagnostic results and performing advanced nursing techniques and critical care procedures

• Identifying and assessing major medical problems and communicating potential needs to a physician/specialist

Qualifications:

Applicants should hold a current RN license and valid driver's license. Vaccination from covid-19 is a condition for hiring. A request for exemption can be submitted and will be reviewed and approved on an individual basis.
