Client Facility Location: Lebanon, PA
Hours: 6:30am-10pm. Flexible shifts within that time range. 12 hour shifts available. Weekdays and weekends available!

Job Summary:
This Intermediate Care Facility (ICF) provides a supportive, home-like environment for 24 individuals with severe or profound intellectual disabilities, offering 24-hour care designed to enhance quality of life through personalized health services, rehabilitation, and daily living assistance.

Team members play a vital role in supporting residents with complex needs, helping them live with dignity, comfort, and as much independence as possible. Three days of comprehensive training are provided to ensure all new staff are prepared and confident in their roles.

The facility also serves individuals with declining physical or mental health, as well as seniors who require daily assistance but do not need skilled nursing care. Services focus on active treatment, including specialized training, healthcare, and nutritional support, all delivered in a compassionate and structured environment.

Requirements:

  • Clearances:
  • FBI Clearance (current within 5 years)
  • PA State Police Clearance (current within 5 years)
  • Child Abuse Clearance (current within 5 years)
  • LPN:
  • Valid LPN license
  • CPR certification
  • Additional Requirements:
  • TB test (w/in 1yr) , Quantiferon Gold (w/in 1 yr) or Chest X-Ray (w/in 5 yr)
  • Physical/fit-for-duty exam (w/in 1 yr)

Qualifications:

  • Licensed Practical Nurse in Pennsylvania (waivered LPN not accepted).
  • Preferred experience working with intellectually and physically challenged individuals.
  • Successful completion of FBI, PA State Police, and Child Abuse clearances.

Working Hours:

  • Part-time: Anything below 40 hours

Physical Demands:

  • Good physical health; ability to lift, bend, stoop, stretch, and assist with tasks.
  • Use of lifting devices when necessary.

Functions:

  • Oversee personal care and ensure a safe, clean environment for individuals.
  • Assist with ADLs, feeding, and positioning.
  • Monitor and record health status; administer medication and treatments.
  • Maintain medical supplies and equipment; assist with physician rounds.
  • Transcribe orders and communicate changes to Health Services Supervisor/Director/QIDP.
  • Update parents on medical changes as needed.
  • Maintain accurate medical records.
  • Participate in admissions, discharges, and transfers.
  • Attend required meetings, in-services, and training sessions.
  • Assist with orientation of new LPNs and provide task direction to Direct Support Aides as needed.
  • Perform other duties as assigned within the Nurse Practice Act.
