Job Title: CHAP-Verified Consultant

Overview:
The CHAP-Verified Consultant provides specialized consulting services to Home Health, Hospice, Home Care agencies, and startups preparing for CHAP accreditation or operational launch. This role supports both established providers and new entrants by ensuring readiness, compliance, and operational efficiency through policy development, staff education, and implementation guidance.

Key Responsibilities:

• Conduct detailed compliance and operational assessments for Home Health, Hospice, Home Care agencies, and startups in alignment with CHAP standards.

• Guide startups through the process of launching compliant operations and preparing for initial CHAP accreditation.

• Review and develop policies, procedures, and clinical documentation to ensure compliance.

• Provide readiness assessments, mock surveys, and corrective action plans.

• Educate leadership and staff on CHAP standards and best practices.

• Develop and implement QAPI (Quality Assessment and Performance Improvement) plans.

• Monitor post-survey follow-up and assist with plan of correction submissions.

• Collaborate with internal consulting and staffing teams to identify opportunities for operational improvement and sustainability.

Qualifications:

• Minimum of 3 years of experience in Home Health, Hospice, Home Care, or healthcare startup consulting.

• Deep understanding of CHAP standards and federal/state regulatory requirements.

• Strong communication, documentation, and presentation skills.

• Ability to work independently and manage multiple clients simultaneously.

Preferred Background:

• Experience as a Home Health or Hospice Administrator, Director of Nursing, or Compliance Director.

• Prior involvement in CHAP or other accreditation surveys.

• Experience supporting startups from licensing through accreditation readiness.

• Bachelor’s or Master’s degree in Healthcare Administration, Nursing, or related field.
