Hospice Community Liaison

Location: Cleveland, Ohio Area

About the Role:
Are you passionate about making a difference in healthcare and building strong community connections? We’re looking for a proactive professional to grow awareness of hospice services and develop lasting relationships with healthcare providers and local organizations.

What You’ll Do:

• Connect with hospitals, clinics, nursing homes, and other community partners to introduce and promote hospice services

• Organize and participate in educational events and outreach programs

• Maintain strong, ongoing relationships with referral sources

• Identify opportunities to expand the agency’s presence in the region

• Work closely with internal teams to support smooth operations and timely communication
   

What We’re Looking For:

• Background in marketing, sales, or community outreach; healthcare experience a plus

• Strong interpersonal, organizational, and communication skills

• Self-starter who thrives independently and can adapt to changing needs

• Professional, approachable, and able to represent the organization in the community

• Valid driver’s license, reliable transportation, and insurance
