Full-Time Salaried & PRN (Per Visit) Opportunities

We are seeking skilled and compassionate Registered Nurses (RNs) to join our growing Home Health team. We are currently hiring for both a Full-Time Salaried RN and PRN RNs (per visit basis) to support patients throughout Montgomery, Philadelphia, Delaware, and Chester Counties.

This role is ideal for nurses who are patient-focused, comfortable working independently in the community, and experienced in home health documentation.

Position Options

1. Full-Time Salaried RN

• Consistent, full-time schedule

• Supports start-of-care visits, ongoing skilled nursing visits, and coordination of care

• Assists with intake and onboarding of new referrals

2. PRN RN (Per Visit)

• Flexible scheduling with visits assigned based on availability

• Paid per completed visit

• Ideal for nurses seeking supplemental income or flexible work

Responsibilities

• Provide high-quality skilled nursing care in patients’ homes

• Perform assessments, start-of-care visits, and routine follow-up visits

• Care for patients with a wide range of needs, including:

  • Trach and ventilator-dependent patients

  • Bedbound patients

  • Wound care and management

• Accurately document all care and visits using Home Care Home Base (HCHB)

• Communicate effectively with the clinical and administrative team

Requirements

• Active Registered Nurse (RN) license in Pennsylvania

• Home Health experience required

• Experience with Home Care Home Base (HCHB) is required

• Ability to travel throughout Montgomery, Philadelphia, Delaware, and Chester Counties

• Strong clinical judgment, organization, and time-management skills

• Reliable transportation
