Registered Nurse (RN) – Home Health (1099, Full-Time Hours)

Our organization is seeking a skilled and compassionate Registered Nurse (RN) to join our growing team as a 1099 independent contractor working full-time hours to support clients primarily throughout Philadelphia and Montgomery County. This role is ideal for a flexible, patient-focused nurse who is comfortable working with a diverse range of patients in the community and assisting with the intake and onboarding of new referrals.

The RN will play a key role in delivering high-quality clinical care while supporting our office with new patient admissions and ongoing case management.

Responsibilities & Qualifications

• Provide safe, competent care to tracheostomy and ventilator-dependent patients

• Support bedbound and medically complex patients in the home setting

• Perform skilled wound care in accordance with physician orders and best practices

• Minimum of one (1) year of clinical or home health experience preferred

• Strong assessment, critical thinking, and clinical judgment skills

• Knowledge of infection control protocols, nursing best practices, and safety standards

• Excellent communication and interpersonal skills with patients, families, and care teams

• Accurate and timely documentation using electronic health record systems
  (Experience with Axxess and/or HHAeXchange is a plus)

• Provide shift-based nursing care and conduct skilled visits as needed

• Open and manage cases to help streamline new patient referral onboarding and intake

• Maintain a flexible schedule while committing to full-time hours

• Willing and trainable to learn and complete OASIS Start of Care (SOC) admissions
